Eastern Area Agency on Aging is currently accepting applications for a 32 – 36 hour per week, Caregiver Specialist. The position will complete an intake/assessment, develop a person-centered care plan, provide educational information, link the caregiver and their care partner to services and programs, and provide ongoing support and triage. The Caregiver Specialist will also answer calls along with the Community Resource Specialists, providing information and assistance to older adults and adults with disabilities regarding state and federal services, benefit programs, and community resources. The Caregiver Specialist will be home based within one of our rural communities (Hancock or Washington Counties), some travel within our four-county coverage area is required.
A bachelor’s degree in Human Services, Social Work, Human Development, Gerontology, or related field required, LSW or previous Case Management experience preferred. Excellent written and verbal communication skills are essential. Database experience is essential.
