Community Living Services/Respite Program
UBHS provides a variety of community living services and supports including but not limited to: adult group homes, home based habilitation homes, residential based supported community living for children, supported community living drop in services, and respite. These services enable individuals with a variety of behavior disorders, disabilities and mental illnesses to participate in the community with the highest possible degree of personal autonomy. These services are person-centered and include opportunities to engage in community life and receive services in the community to the same degree as individuals who do not receive services.
Job Description: The Support Care Coordinator is responsible for assisting and supporting consumers in the UBHS Community Living Supports/Respite Care program in reaching their self- determined goals, and performs other duties as assigned.
Key Responsibilities:
Provide services to consumers in the CLS/Respite program through the implementation of their Individual Service Plans (ISPs) within the community.
Participate in the ISP process as necessary.
Create and maintain records of individual care, progress, issues, and/or concerns.
Report and discuss observations with the Program Manager and coordinating clinician.
Ensure the health, safety, and welfare of individuals in the program.
Assist individuals in achieving personal goals.
Assist individuals with daily living functions such as coping skills, laundry, meal planning, personal hygiene, shopping, and recreation/leisure skills.
Provide transportation within the community for activities.
Complete incident reports as required.
Other duties as assigned.

Qualifications:
Interpersonal skills and ability to work with a variety of people to help them achieve their personal goals.
Ability to act responsibly in a crisis and use good judgment.
Ability to train, teach, and support others in learning daily living skills such as cooking and household chores.
Ability to work in/out of the home with a range of various functioning levels.
Experience working with individuals with disabilities preferred.
