Position Title: Admissions Liaison
Exempt Status: Full-Time Exempt
Wage Rate: Up to $75k based on experience *With commission potential
Work Schedule: Monday- Friday (On-call responsibility)
Sedgebrook is recruiting for a hospitality focused Admissions Liaison to join our team. This position is responsible for managing the Health Center admissions and referral process and for establishing and maintaining an open, effective communication between the facility and referral sources, physicians, and third-party payers.

- Responds to referrals in a timely manner, utilizing the stop light grid (green/yellow/red)
- Follows pre-admission guidelines for new admissions to prepare the internal team on anticipated medical needs or anything else necessary for ensuring a smooth transition.
- Partners daily with internal community admissions, clinical, and operations team to ensure communication on pending referrals, admissions and other pertinent data related to admissions to the skilled community.
- Reviews clinical documentation including: H&P, therapy, labs, MAR, and other relevant items, in order to make admission decisions.
- Follows up on Sedgebrook/Radford Green community residents in the hospital to triage to skilled as appropriate, or communicate and hand off to appropriate community for a smooth return.
- Maintains all professional contacts and assessments in company preferred database/EHR system.
- Creates and maintains relationships with local acute hospitals, other area Communities, hospices, Home Health agencies, and other referral partners, etc. to build partnerships that result in direct admissions.
- Identifies and coordinates outreach opportunities for Health Care Center participation, such as but not limited to trade show booths, civic participation and others as identified.
- Assists in maintaining stock of promotional items for outreach, fairs, and trade expositions; and re-orders when needed.
- On call as necessary, and participates in the weekend rotation, as necessary
- Bachelor's degree preferred
- Clinical background preferred: RN, LN, PT, OT, ST, and/or Clinical Social Worker
- Minimum of 3-5 years of experience in Admissions, Healthcare, and/or Skilled Nursing
- Must have strong time management and organizational skills, be able to relate to all levels of management/professionals and be a self-starter, goal oriented, team player.
