Overview:
We are seeking a compassionate individual to join our team as a Direct Support Professional. As a Direct Support Professional, you will help our Clients manage their home health care plan.

Responsibilities:
- Provide orientation training and complete enrollment forms in approved format in a timely manner.

- Assist Clients in the community through home visits and community meetings.

- Provide support and education to Clients to assist them in adhering to home care rules & regulations.

- Strive for high customer satisfaction by providing prompt, solution-focused support to Clients and Social Services.

- Complete all telephone and face to face contacts with patients in a timely and professional manner.

- Recruit new Clients into the home care program to provide them with personal care assistance at home.

- Assist Clients in accessing and understanding necessary health and wellness information provided by their medical provider.

- Maintain confidentiality of all clients information

Experience:
- Experience working with underserved and diverse populations in community settings required

- Experience working with elderly and people with disabilities is required

- Knowledge or experience in social work, or caregiving is a plus

- Ability to drive and valid driver's license required

- Strong communication and interpersonal skills

- Experience in caregiving roles is beneficial

Job Types: Full-time, Part-time

Pay: $15.00 - $18.00 per hour

Expected hours: 20 – 30 per week
