LICSW

Responsibilities:
1. Apply social work theory, knowledge and methods to improve the social condition of individuals, families through the provision of services other than counseling or psychotherapy.
2. Obtain information from agency clients and/or applicants for services to determine their service needs and/or the availability of appropriate resources.
3. Assist individuals, families, and or groups in identifying, locating, arranging, and making use of resources and services that are available to resolve day-to-day problems (e.g., employment, housing, health care, financial assistance problems, placement resources, etc.).
4. Assist individuals, families in long term planning and assistance for decision making.
5. Provide information on Assisted Living/Nursing home care/placement information.
6. Evaluate patient progress towards goals and advise care plan accordingly in consultation with appropriate individuals, team members and/or social work manager.
7. Work collaboratively in communication with multidisciplinary team to support care plan and team goals.
8. All documentation meets the needs of Medicare, DPH and agency requirements and is completed and transmitted within 24 hours according to policy.
9. Maintain current knowledge base on social work issues through conferences, educational requirements for licensure, and social work literature.
10. Perform miscellaneous job-related duties as assigned.
