Radias Health is Hiring a Program Director Near Saint Paul, MN

RADIAS Health is seeking an experienced, creative, full-time Mental Health Professional to be the Program Director for two small programs. This positon would have both direct client service time and be responsible for supervising staff. Whatever IT Takes (WIT) specializes in assisting individuals with serious and persistent mental illness discharged from state institutional settings and back into the community. Community Care Service Team (CCST) is a program in partnership with Health Partners that specializes in supporting individuals who have a history of high utilization of emergency or inpatient services. This person must have excellent clinical and problem solving skills, along with a good understanding of community resources in the metro area. Candidate will meet the qualifications as a Mental Health Professional with a LICSW, LP, LPCC or LMFT license in the state of Minnesota.
Hours are Monday - Friday, day hours.

Purpose
To provide clinical, recovery-based transition services to expedite discharge from Anoka Metro Regional Treatment Center and the Minnesota Security Hospital for individuals deemed clinically ready for discharge by overcoming client-specific barriers to discharge. To develop individual community reintegration plans that are sustainable and ensure successful community living for the individual. The Director will supervise a team of multidisciplinary practitioners and will partner with counties, tribes and other community providers to leverage existing resources and/or develop new services. To provide clinical, recovery-based services to support individuals who have a history of high utilization of emergency and inpatient service and have Health Partners PMAP (Prepaid Medical Assistance Program) to remain in the community.
Essential Functions
• Provide courteous and respectful care to all clients.
• Responsible for all clinical, organizational, and financial aspects of the programs.
• Develop case conceptualizations based on record reviews, client interviews and consultation with providers in order to develop community reintegration plans that leverage strengths, mitigate barriers, and are based on individuals' needs and choices.
• Coordinate clinical transition plans with the county, state, and involved providers and procure the identified resources necessary for discharge and successful living in the community.
• Act as a liaison to counties and the MN Department of Human Services with all clinical, contractual, and funding matters.
• Provide clinical direction to team members for the day-to-day clinical operations.
• Coordinate program activities with community providers and other agencies/entities to ensure optimum level of communication and care of the client.
• Establish and maintain professional relationships with the State (AMRTC, St. Peter, DHS) and County providers to ensure an accurate understanding of the WIT grant and appropriate use of services//funding in client discharge planning.
• Establish and maintain professional relationships with Health Partners leadership to ensure accurate understanding of the contract deliverables.
• Provide psycho-education to community providers/family to help ensure that they can effectively support each client on an ongoing basis in the community.
• Ensure standards are met according to State and County guidelines. Ensure appropriate reporting is submitted to Vulnerable Adult, Child Protection, the Ombudsman’s Office, or other agencies/entities when necessary.
• Meet regularly with State and County administration on program development and coordination.
• Develop and administer programs budget including line items, and to cover travel and transportation costs.
• Develop and maintain program policies and procedures and revise as necessary.
• Ensure that required program evaluations are completed.
• Develop and ensure completion and compliance of all mandated training. Identify and/or conduct any necessary individual training for direct reports.
• Responsible for keeping on-going performance documentation for all direct reports.
• Maintain a pattern of regular work hours.
• Hiring of all positions in concert with Human Resources Manager and Recruiter.
• Must be able to transport clients in the community.
• Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
• Participate actively in on-going professional growth and development, maintain appropriate professional behavior, and participate in appropriate supervision.
• Maintain a pattern of regular work hours to meet program needs as determined by the Associate Director of Clinical Services.
• Other duties as assigned.
Qualifications: Education & Experience
• Masters or Ph.D. in the Behavioral Sciences
• Minimum of five year’s work experience providing services to individuals with mental illness.
• Supervision experience desired
• Licensed Mental Health Professional in the State of Minnesota
CEU Requirements
• Must maintain required continuing education units according to individual professional license.
• Must complete 2 Human Resource trainings annually.
Competencies
• Have excellent clinical assessment skills.
• Have expertise in evidence based practices.
• Have expertise with assessment, treatment planning and treatment delivery for individuals with serious and persistent mental illness and co-occurring substance use disorder.
• Have knowledge and experience in working with individuals with co-morbid physical health issues.
• Be knowledgeable about and utilize various substance abuse treatment modalities such as harm reduction and motivational interviewing.
• Have demonstrable working knowledge of vulnerable adult laws and other pertinent rules and regulations.
• Knowledge of current principles, practices and techniques of case management.
• Knowledge of symptoms and course of serious and persistent mental illness as well as medications and other therapies.
• Knowledge of community resources.
• Knowledge of supportive counseling techniques, independent living skills/teaching and crisis intervention techniques.
• Assess and manage crisis situations. Must have good administrative, organization, and supervisory skills and the ability to think creatively about discharge planning and community reintegration.
• Utilize crisis assessment tools for suicidal and aggressive behavior.
• Develop appropriate therapeutic relationship with clients.
• Ability to work independently and as a member of an interdisciplinary team.
• Establish and maintain relationships with all levels of staff.
• Able to problem solve using analytical skills, creativity and logical thinking to reach a solution.
• Able to work under pressure in a fast paced time sensitive environment with shifting priorities and multiple deadlines.
• Multitask in an environment with multiple distractions is a must.
• Able to use a computer for word-processing and data base management.
• Be able to speak, clearly understand, and write the English language.
• Must have a car, a current and valid driver’s license, valid auto insurance and be able to drive during the day or evening hours that may require you to drive in the dark.

Work Environment:
This position operates in a professional office environment. Occasionally exposed to a variety of different work conditions including but not limited to, being outdoors during inclement weather, construction sites, exposure to excessive temperatures hot/cold.
