BASIC FUNCTIONThe Health Information Associate is responsible for the retrieval, distribution, scanning and posting of medical records and reports. The Health Information Associate works to provide medical information to the health care team, vital to providing quality patient care.
PRINCIPLE DUTIES
  • Distributes the daily records and reports to the appropriate providers within the clinic.
  • Scans all health care records and reports from outside facilities.
  • Posts the scanned documents into the appropriate categories within the correct patient’s electronic health record.
  • Abstracts and enters data into the electronic health records.
  • Obtains immunization records, hospital records and records as requested.
  • Completes all Release of Information requests for outside entities:
    • Requests for electronic records will be completed within 3 business days.
    • Requests for paper records will be completed within 10 business days.
  • Employees are responsible are for maintaining a thorough understanding of their role in the following protocols and as protocols are updated periodically employees are expected to remain current on any updates.
    • Patient Record Requests
    • Medical Records Scanning
    • Dental Records Scanning
    • Disposal of PHI (Excluding Patient Charts)
    • Decedent PHI Disclosures
    • Accounting of Disclosures
  • Acknowledges that White House Clinics strive to be a patient-centered medical home and, as such, prescribe to illustrates the employee’s role in aforementioned team-based care and accepts responsibility for being an active member of the team, including identification of quality improvement opportunities.
  • Assists with other duties or in other departments as instructed by supervisor.
Care Coordinator BASIC FUNCTIONCare Coordinators are responsible for reporting and organizing patient data to highlight care gaps. The Care Coordinator is vital to the care team in managing chronic disease within the patient population as they work to identify activities to improve health outcomes for patients of White House Clinics.
PRINCIPLE DUTIES
  • Responsible for the comprehensive and accurate completion of the Care Coordinator Worksheets related to the proactive office encounter model of care.
  • Prior to a patient’s medical appointment, the Care Coordinator will:
  • Print a list of each day’s appointments, for all assigned providers.
  • Add all appropriate Care Guidelines to the patient’s NextGen health record.
  • Using data mining and abstraction, accurately identify which health measures have been appropriately completed.
  • Identify which health measures are outstanding and necessitate completion on the Care Coordinator Worksheet based on guidelines set forth organizationally.
  • Provide the Care Coordinator Worksheet to the medical care team prior to the day’s visit.
  • Actively participate in medical care team huddles prior to the day’s appointments.
  • Employees are responsible are for maintaining a thorough understanding of their role in the following protocols and as protocols are updated periodically employees are expected to remain current on any updates.

QualificationsSuccessful applicants will be mature and well organized. Candidates should have knowledge of HIPAA regulations relating to PHI and release of PHI as well as working knowledge of computer basics including Microsoft Excel and Word programs. Candidate should possess ability to effectively communicate with a variety of colleagues. Successful applicants will have strong organization skills, attention to detail, ability to multitask, and meet deadlines.

Requires high degree of initiative and ability to complete work with limited oversight. Qualified candidates must be adept at prioritizing tasks to optimize office flow and to meet deadlines. Provide superior customer service to all patients, external customers (office staff of other offices, hospital, community agencies, etc.), and co-workers. Serves as an ambassador of the White House Clinics in all conversations and interactions with these parties.
ServiceDemonstrates excellent customer service through his/her attitude and actions, consistent with the standards contained in the Mission and Vision and Values of the organization. Maintains clinic standards for a clean and quiet patient environment to maintain a positive patient care experience. Promotes culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
ComplianceMaintains compliance with organization’s policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards. Complies with organizational and regulatory policies for handling confidential patient information. Adheres to professional standards, clinic policies and procedures (OSHA, HIPAA, Standing Orders, etc.), federal, state, and local requirements, Health Resources and Services Administration (HRSA), U.S. Department of Health and Human Services (HHS), Office of the Inspector General (OIG), etc. Cooperates fully with all aspects of Corporate Compliance Plan and Standards of Conduct. Participates in all safety programs which may include assignment to an emergency response team.
ACCOUNTABILITYPosition is accountable to the Health Information Management Service Line Manager. From time to time, the employee may be directly accountable to the Chief Operating Officer for duties associated with special programs or assignments.
SUPERVISION EXERCISEDNone.
TYPICAL PHYSICAL DEMANDSThis position requires sitting, some bending, stooping and stretching. Eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment are also required. Position requires normal range of hearing and eyesight to record, prepare, and communicate appropriate reports. Employees will be required lift papers or boxes up to 50 pounds occasionally.
TYPICAL WORKING CONDITIONSWork is performed in office environment and involves frequent contact with staff and the public. Position may involve dealing with angry or upset people. Evening and/or weekend work is required. Work may be stressful at times.
Travel RequirementsRegular travel is not a requirement for this position. Employees may be requested to provide coverage for another clinic on occasion.
SALARY$15.04 - $23.68
WORK HOURS37.5 hours (Irregular work hours, some overtime possible at times.)
EVALUATIONEvaluated annually by the Team Leader or Service Line Manager with input from providers and support staff as appropriate.
