Resorts at Chester River Manor is Hiring a Social Services Designee Near Chestertown, MD

Respectfully, the Resorts at Chester River Manor is in immediate need of a Social Service Designee. The Designee is responsible for promoting the psychosocial and overall wellbeing of Resorts at Chester River Manor residents. This entails providing case management and other related support in all levels of care, enabling continuity of care for residents and families, and meeting regulatory requirements for licensed nursing beds in the facility. The Social Service Designee is also responsible for continuity of care to all residents for whom he/she is the primary designee. Essential Social Service Designee Functions

  • Responsible for delivery of social services to residents and their families.
  • Will be expected to be readily able and receptive to work with patients\residents, families, and doctors.
  • Assists in preadmission and admission interviews, informing resident and family of available services and resident rights and responsibilities.
  • Assesses residents’ needs and provide case management and support to promote and maintain optimal quality of life and wellbeing.
  • Assesses changes in resident’s psychosocial health and assists residents with transition and brief crisis intervention.
  • Supports aging in place at all levels of the continuum; provides timely assessments, counseling, education, support services, and referral.
  • Prepares social histories for use by professional facility staff and participates in resident care conferences.
  • May be involved in in-service training of aides and staff related to resident rights and social services.
  • Coordinates care plans, assesses changes in resident's psychosocial health and assists resident with transitions and crises.
  • Record new admissions and discharges per daily census report.
  • Setup care plans in PointClickCare (PCC) for new admissions and complete care plans on all that are due per listing.
  • Must be competent with and responsible for ensuring the completion of MDS Sections: C*- Cognitive Patterns, D- Mood, E- Behavior, Q*- Participation in Assessment and Goal Setting
  • Prepare PASRR (Preadmission Screening and Resident Review) ID & SMI screens for new admissions and record MOLST (Maryland Medical Orders for Life-Sustaining Treatment) designation
  • Attend all care plan meetings and at month’s end complete the quality-of-life report.
  • Coordinator will be responsible for recording resident initial BIMS (Brief Interview for Mental Status) and PHQ-9 (Resident Mood Interview) score, Advanced Directive/Living Will
  • Must be familiar with adult waiver program
  • Completes appropriate documentation upon admission, transfer, and discharge. Also, must process guardianship paperwork as required.
  • May communicate with residents and families and assist with finding appropriate resources within the community.
  • Handles problems of adjustment to long term care facility environment.
  • Assists with arrangements, if needed, for out of facility healthcare, transportation, and counseling services. Handles discharge planning.
  • Communicates with Interdisciplinary Team regarding admissions, transfers, and discharges, scheduled family meetings, and other issues of common concern.
  • Advocates for Residents Rights including abuse, neglect, exploitation and/or psychosocial/spiritual needs.
  • Coaches, supports, and enables other team members to respond to resident’s emotional and/or psychological needs.
  • Other duties as assigned.

Required Knowledge, Skills, and Abilities

  • Knowledge of long-term care facility environment and the effects of disease and disability upon the resident and the resident's family.
  • Knowledge of available community resources.
  • Ability to work independently and exercise good judgment in problem solving and networking within the facility and community.
  • Interest in and appreciation of older adults and chronically ill individuals.
  • Positive, pleasant, understanding attitude.
  • Ability to communicate effectively with good verbal and writing skills.

Qualifications

Designee:

Graduation from high school with at least three years experience in some capacity working with older adults, with at least one year of the experience being in a long-term care facility. College course work in sociology, psychology, gerontology, or social work may be substituted for up to two years' experience

Job Type: Full-time

Pay: Up to $25.00 per hour

Expected hours: No less than 40 per week
