DESCRIPTION: The Community Outreach Associate will support Healthix’s efforts to promote MyHealthRecordNY.

About MyHealthRecordNY: MyHealthRecordNY is a secure online patient portal that is available to anyone who has received care in New York State. It provides patients with access to their health information if their care provider shares data with Healthix, the New York City/Long Island Health Information Exchange (HIE), or Hixny, the HIE serving the greater Capital Region. Healthix has partnered with Hixny and CLEAR the trusted identity verification service, to present a consolidated view of patient data while ensuring secure and verified access for patients aged 18 and older.

We are seeking a proactive and engaging Community Outreach Associate to join our team to promote MyHealthRecordNY. This role involves attending health fairs, community centers and other community events in Brooklyn, Queens, Bronx & NYC to inform residents about the benefits of My Health Record NY and assist them in creating their accounts and accessing their health information online. The ideal candidate is passionate about health care accessibility, tech-savvy, results-driven and enjoys interacting with the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

•Event Participation: Attend health fairs, community centers, including Senior Centers, events, and other outreach activities to promote MyHealthRecordNY.

•Community Engagement: Engage with New York residents aged 18 and older to share information about the patient portal.

•Achieving Goals: Expected to enroll a minimum of 50 or more individuals per week.

•Education and Assistance: Educate community members on the benefits and functionalities of the MyHealthRecordNY portal. Assist residents with the registration process, including providing access to QR codes and helping them navigate the portal.

•Material Distribution: Hand out flyers, brochures, and other informational materials.

•Contact Information Collection: Collect contact information from interested residents to facilitate continued outreach and engagement.

•Collaboration: Work closely with the outreach team and community partners to coordinate efforts and maximize impact.

•Reporting: Maintain records of outreach activities and provide regular updates on engagement outcomes.

•Attend onsite and web meetings as needed.

QUALIFICATIONS:

•Experience: Prior experience in community outreach, health promotion, or a related field is preferred.

•Communication Skills: Excellent verbal and written communication skills with the ability to effectively convey information to diverse audiences.

•Interpersonal Skills: Strong interpersonal skills with the ability to build rapport and engage with community members.

•Tech Savvy: Comfortable using digital tools and assisting others with online registrations and QR codes.

•Organizational Skills: Highly organized with the ability to manage multiple tasks and events simultaneously.

•Passion for Health Care: A genuine interest in improving health care access and empowering individuals with their health information.

•Flexibility & Reliability: Willingness to travel to various locations within New York State for events and outreach activities.

•Professionalism:Represent Healthix as ambassador of Healthix values and reputation as the state’s leading HIE.
