Job Description
The Clinical Care Team supports primary care providers by coordinating social services, patient resources, and care navigation. Responsibilities include:
Social Support & SDOH Coordination
- Navigate social determinants of health (SDOH): food insecurity, housing, transportation, etc.
- Maintain an updated community resource list (eligibility, referrals, contacts).
- Conduct in-person or virtual social needs assessments for referred patients.
- Coordinate access to social service programs and community resources.
- Collaborate with clinical staff, nurses, and other social workers.
Case Management Duties
- Support discharge planning, follow-up, and patient education.
- Assist with retrieving patient records from hospitals and specialists.
- Help secure medical equipment through community partners.
- Track care plans, follow up on overdue or lost-to-follow-up patients.
Specialty Referral Navigation (as needed)
- Schedule and track specialist and imaging referrals.
- Obtain records from specialists and imaging centers.
- Maintain a resource list of specialty referral options with eligibility, processes, cost, and contacts.
Prescription Assistance
- Help patients access low-cost medication programs.
- Complete applications for programs like CoverRx, RxOutreach, and patient-assistance programs.
- Serve as liaison between patients, providers, and pharmaceutical programs.
Additional Responsibilities
- Assist with primary care initiatives that include social work components.
- Document thoroughly in patient records and update consults as needed.
- Provide or source patient education materials.
Expectations
- Complete all onboarding and orientation requirements.
- Participate in regional and primary care meetings.
- Attend provider meetings and community health councils.
- Build partnerships with social service agencies.
- Identify patient barriers to care and recommend solutions.
