Job Summary
We are seeking an experienced Clinical Hospital Pharmacist for a 13-week contract assignment in Zanesville, Ohio. The ideal candidate will have inpatient hospital pharmacy experience, strong clinical knowledge, and expertise in pharmacokinetic dosing, particularly vancomycin, and warfarin management.
The pharmacist will work varied day and evening shifts, including weekends and holidays, to support safe and effective medication management within a hospital setting.
Responsibilities
- Review and verify medication orders for accuracy, safety, and appropriateness.
- Provide clinical pharmacy services to inpatient hospital patients.
- Perform pharmacokinetic dosing and monitoring, particularly for vancomycin.
- Manage warfarin therapy and monitor patient responses.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy.
- Monitor drug interactions, contraindications, and potential adverse effects.
- Document clinical interventions and medication-related activities in the electronic medical record.
- Utilize the Epic electronic medical record system for pharmacy-related tasks.
- Provide coverage for evening shifts, weekends, and holidays as required.
Requirements
- Active Ohio Pharmacist license in good standing — mandatory.
- Minimum of 1 year of inpatient hospital pharmacy experience.
- Experience with pharmacokinetic dosing, especially vancomycin.
- Experience with warfarin management.
- Ability to work varied shifts, including evenings, weekends, and holidays.
- Strong clinical judgment, communication, and medication management skills.
- Epic Willow Inpatient experience preferred but not required.
Pay: $3,000.00 - $3,300.00 per week
