Duties
- Observe and monitor patients’ conditions, recording vital signs and reporting changes promptly to healthcare professionals
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Support with medical procedures such as catheter care, medication administration, and wound care following home healthcare regulations
- Operate Electronic Health Record (EHR) systems to document patient information accurately and maintain confidentiality in compliance with HIPAA standards
- Facilitate patient transfers using safe methods such as Hoyer lifts or patient transfer techniques while ensuring safety for both client and caregiver
- Provide companionship and emotional support to individuals with dementia or Alzheimer's disease, including memory care activities and engagement strategies
- Assist with meal preparation, light housekeeping, laundry, and maintaining a clean and safe environment for clients
- Follow emergency protocols by administering first aid or CPR when necessary and maintaining readiness for urgent situations
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery and uphold patient rights
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare environments preferred but not required; training provided for motivated candidates
- Demonstrated ability to work with geriatric patients or individuals with developmental disabilities and dementia care needs
- Knowledge of medical terminology, patient care procedures, and home health care regulations is highly desirable
- Experience operating EMR/EHR systems for accurate documentation of medical records and patient data
- Familiarity with patient transfer methods such as Hoyer lifts or other mobility aids; ability to perform heavy lifting safely when needed
- Certification in CPR and First Aid is required; additional certifications in hospice or end-of-life care are a plus
- Strong communication skills, compassion for patients, and a proactive attitude are essential for success in this role
