Position: Case Manager
Location: Randolph, NJ
Schedule: Monday – Friday, 11:00 AM – 7:00 PM

What You’ll Do
- Provide case management services aligned with individualized treatment plans
- Help clients access essential resources such as housing, healthcare, employment, and financial support
- Collaborate with clinical teams to coordinate and enhance client care
- Maintain knowledge of community resources and connect clients with appropriate services
- Schedule and manage client appointments based on individual needs
- Complete required documentation, including NJSAMS reporting (admissions, updates, discharges, etc.)
- Support intake processes and assist with program operations as needed
- Communicate effectively with clients, families, and external providers
- Ensure all documentation meets regulatory and organizational standards
What We’re Looking For
- 1–2 years of experience in case management or a related field
- Reliable transportation
- Strong organizational and communication skills
- Ability to work collaboratively while managing responsibilities independently
- Compassionate, patient, and professional approach to client care
Compensation & Benefits
- Salary: $55,000 – $65,000 (based on experience)
