This role integrates assessment, individualized treatment planning, counseling, and interdisciplinary collaboration to address psychological and emotional factors, promote measurable outcomes, and ensure high-quality, ethical care. The Mental Health Counselor is an onsite position.
Essential Functions
- Conduct comprehensive client intakes, psychosocial assessments, and diagnostic evaluations to identify behavioral, emotional, and psychological barriers and determine appropriate services.
- Develop and implement individualized, goal-oriented treatment plans using evidence-based approaches in collaboration with clients and interdisciplinary team members, including coordinating referrals as needed.
- Provide individual and group counseling services through in-person and telehealth modalities to address mental health, behavior change, and weight management–related factors.
- Monitor client progress by evaluating measurable outcomes, adjusting treatment plans accordingly, and maintaining accurate clinical documentation in compliance with HIPAA.
- Facilitate therapeutic groups and collaborate with interdisciplinary teams while engaging in ongoing professional development to maintain licensure, clinical competency, and quality of care.
- Prepare, prevent, protect and respond to support our culture of safety.
- Works hours per week in accordance with offer letter or per attributes of position at transfer request. May be expected to work longer hours as necessary.
