What are the responsibilities and job description for the Foster Care Family Services Case Manager position at WOLVERINE HUMAN SERVICES?
RESPONSIBILITIES

This hybrid roll will provide Foster Care Case Management and Permanency planning services to assigned recipients of care, birth, relative and foster families and provide direct licensing case management services, placement interventions, emergency response, and training to children and adults involved with Michigan’s child welfare system.

Act as an advocate, liaison and or mentor on behalf of the WHS, clientele, contractor, court and or community partners in various locations and settings.

Responsible for the provision of quality Case Management performance. Must successfully meets and or exceeds all licensing, contractual and agency performance standards.

Assess families and individuals in regard to their ability to effectively foster, care for and or adopt recipients of care.

Travel throughout the state of Michigan and to utilize a personal vehicle and or agency vehicle (when available) in order to successfully complete contractual and agency assignments/tasks.

Support the Wolverine Human Services Mission Statement and present the agency and its clientele in a professional, ethical and positive fashion.

Adhere and follow all case related court orders.

Maintain a professional, supportive, confidential, and therapeutic atmosphere for our clientele, co-workers, community partners.

Must be available and willing to respond to agency/recipients of care related needs on a twenty-four-hour basis.

Participate in Quality Assurance activities. Assist with the compliance of performance based corrective action plans (CAP’s).

Completes all documentation in the time frame required by DHHS, the court and Wolverine Human Services. Produce all required licensing contractual and agency work related to reports, incident reports, documentation, family assessments, home/family studies etc.

Conduct all assigned special investigation, develop and implement corrective action plans, regulate and enforce all licensing and contractual rules. Adhere and follow all case related court orders.

MINIMUM QUALIFICATIONS

Master's degree from an accredited school of Human Behavioral Sciences such as Social Work or Psychology preferred, bachelor’s degree required. Must meet the required experience as specified within the state of Michigan’s licensing and contractual rules and expectations.

Working knowledge of Michigan’s Child Welfare system preferred.

Excellent written and oral communication and organizational skills.

Valid Michigan drivers and chauffeur’s license or ability to obtain chauffeur’s license.
Salary : $45,000 - $50,000
