We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our dynamic healthcare team. As part of one of New York's premier community health networks, you will provide high-quality, comprehensive primary care to patients of all ages in a supportive, team-based environment.
Position Highlights:
100% Outpatient Primary Care
Key Responsibilities:
Conduct thorough health assessments and manage both acute and chronic conditions
Develop, implement, and evaluate personalized treatment plans
Prescribe medications and therapies in accordance with state regulations and clinical guidelines
Collaborate closely with physicians, nurses, and support staff to ensure coordinated, patient-centered care
Educate patients and families on preventive care, disease management, and healthy lifestyles

Schedule:
Monday to Friday: 8:00 AM – 4:30 PM or 8:30 AM – 5:00 PM
One evening per week: 12:00 PM – 8:00 PM
One weekend day (Saturday or Sunday) per month
Patient Load: ~16 patients per day
