- Provide outreach and housing-focused case management for individuals living with HIV/AIDS.
- Conduct initial HOPWA interviews and develop independent living plans for eligible clients.
- Coordinate comprehensive services for clients receiving rental subsidies.
- Conduct required housing inspections to ensure program compliance.
- Manage referrals for housing services and emergency financial assistance.
- Perform outreach to high-risk populations and connect individuals to prevention and support services.
- Maintain thorough documentation of client interactions, services provided, and program activities.
- Participate in regular supervision, required trainings, and scheduled outreach events.
- Build and maintain collaborative relationships with community organizations and service partners.
- Perform additional duties and support initiatives as assigned.
- Bachelor’s degree in a social-service field preferred; an associate’s degree or a high school diploma with two years of relevant paid experience is also acceptable.
- Must be free of communicable diseases and complete biannual Fitness for Duty assessments, including required immunizations.
- Must have an unrestricted, valid Connecticut driver’s license, reliable transportation, and current insurance—or reliable transportation sufficient for travel between sites; motor vehicle records are checked annually.
- Completion of Basic Annual Safety Training is required.
