Job Function
- Conduct outreach to high-risk populations and connect individuals to appropriate prevention services.
- Build and maintain strong partnerships with community agencies.
- Help organize and participate in prevention-focused events.
- Collaborate with internal clinical staff to support effective opt-out HIV screening.
- Provide biomedical education related to HIV, HCV, STIs, and preventive services.
- Support integration of routine testing within clinical workflows.
- Collect and accurately document client and program data, including counseling sessions, testing, interventions, referrals, and progress notes.
- Complete required data entry and reporting for EvaluationWeb, E2CT Prevention, and other required systems.
- Attend all required trainings, supervision sessions, and professional development activities.
- 2 -5 years prevention experience coming from NFP or OPWDD.
- Bachelor’s degree in a social-service field preferred; an associate’s degree or a high school diploma with two years of relevant paid experience is also acceptable.
- Must be free of communicable diseases and complete biannual Fitness for Duty assessments, including required immunizations.
- Must have an unrestricted, valid Connecticut driver’s license, reliable transportation, and current insurance—or reliable transportation sufficient for travel between sites; motor vehicle records are checked annually.
- Completion of Basic Annual Safety Training is required.
