Hospice Master of Social Work

We are seeking a Hospice Master of Social Work (MSW) to provide coverage in Victoria, Goliad, Beeville, and Refugio, Texas.

White Orchid Hospice is an Medicare & Medicaid certified healthcare organization and it is our mission to assist patients and their families with the inevitable symptoms of a terminal illness. Our goal is to improve the quality of life of a patient's last days and to offer comfort and dignity.

White Orchid Hospice is currently seeking a Part Time Social Worker for the greater North Houston metropolitan area.

Duties include: making home visits and nursing facility visits, admission assessments, pre-bereavement assessments and attending required IDT meetings.

Qualifications

Applicant must possess a Bachelor's or Master's degree in Social Work from an accredited school. Applicant also needs reliable transportation to travel the service area and a Smart Phone to access a HIPPA compliant texting application.

Knowledge needed of community resources on how to make appropriate referrals to community and other relevant agencies for services and ability to coordinate the services. Capacity needed to independently assess the psycho-social functioning and needs of patients and their family members and to formulate and implement a treatment plan, by foregoing alliance with the patient, family and interdisciplinary treatment teams. Capacity needed to independently conduct psycho-social assessments and provide psycho-social treatment to hospice patients with diversified backgrounds.

Requirements

• 2 years of Hospice Experience
• LMSW Required
• Current CPR
• Reliable Transportation
• Kinnser EMR experience preferred but not required
• Demonstrates good verbal and written communication, and organization skills.
