Job Summary:
Medical social services will be provided by a qualified social worker, under the direction of a physician. Social work services are based on the patient’s psychosocial assessment and the needs of the patient and family. The social worker participates in care coordination with the Interdisciplinary Group/Team (IDG/IDT) and on-call rotation per Hospice needs.
Additionally, the MSW may provide leadership and mentorship to other social workers, ensuring high-quality care delivery and supporting professional development within the team.

Job Qualifications:
- Licensure: Current and valid license as an LMSW issued by the Texas State Board of Social Worker Examiners.
- Education: Master’s degree in social work (MSW) from an accredited institution.
- Experience: Minimum 1 year in palliative care or hospice preferred. Supervisory or leadership experience in healthcare is a plus.
- Proficiency in electronic health record (EHR) systems (HCHB experience preferred)
- Strong interpersonal and communication skills
- Ability to analyze data and generate reports for care planning and quality assurance
- Skills in communication, documentation, assessment, and social work methods and techniques.
- Ability to read, write, and speak the English language.
- Reliable transportation. Valid and current auto liability insurance.
Essential Functions:
- Assess and complete comprehensive psychosocial evaluations within five days of hospice election
- Develop care plans addressing psychosocial, financial, environmental, and community needs
- Provide counseling, crisis intervention, and access to community resources
- Collaborate with interdisciplinary teams to create holistic care strategies
- Facilitate training and professional development for social work staff
- Monitor and evaluate effectiveness of interventions
- Assist with advance directives, funeral planning, discharge planning, and changes in level of care
- Promote hospice philosophy and administrative policies
- Attend IDG/IDT meetings and participate in care planning
- Maintain compliance with continuing education and licensing requirements
- Demonstrate commitment to professional growth and competency
- Provide an environment that respects patient privacy and property
- Participate in Hospice QAPI and educational in-service programs
