Responsibilities

• Conduct thorough patient assessments to evaluate health status, including vital signs, physical examinations, and medication reconciliation.
• Develop and implement individualized care plans aligned with medical orders and patient needs, ensuring adherence to all regulatory standards.
• Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols.
• Provide specialized nursing interventions such as wound care, ostomy management, catheter care, and tube feeding.
• Educate patients and their families on disease management, medication administration, health coaching, and lifestyle modifications to promote optimal health outcomes.
• Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities and medical records.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate case management and discharge planning.

Qualifications

• Valid RN license in good standing with current state registration.
• Demonstrated experience in home health care or related settings such as hospitals, nursing homes, assisted living facilities, or hospice environments.
• Proficiency with EMR/EHR systems including Epic or Cerner; familiarity with ICD-9/10 coding is preferred.
• Critical care experience including ICU or emergency medicine is highly desirable; experience with trauma centers or Level I/II trauma center environments is a plus.
• Strong knowledge of medical terminology, physiology, infection control practices, and aseptic techniques.
• Ability to perform comprehensive patient assessments and develop tailored care plans effectively.
• Excellent communication skills for triage calls and working with individuals with disabilities or complex medical needs.
• Certifications such as Basic Life Support (BLS), IV insertion, phlebotomy, and wound suturing are advantageous; additional skills in dialysis, cardiac catheterization support, or pain management are a plus.
