Responsibilities
- Conduct thorough patient assessments to evaluate health status, including vital signs, physical examinations, and medication reconciliation.
- Develop and implement individualized care plans aligned with medical orders and patient needs, ensuring adherence to all regulatory standards.
- Administer medications, including injections and IV infusions, following aseptic techniques and safety protocols.
- Provide specialized nursing interventions such as wound care, ostomy management, catheter care, and tube feeding.
- Educate patients and their families on disease management, medication administration, health coaching, and lifestyle modifications to promote optimal health outcomes.
- Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities and medical records.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate case management and discharge planning.
Qualifications
- Valid RN license in good standing with current state registration.
- Demonstrated experience in home health care or related settings such as hospitals, nursing homes, assisted living facilities, or hospice environments.
- Proficiency with EMR/EHR systems including Epic or Cerner; familiarity with ICD-9/10 coding is preferred.
- Critical care experience including ICU or emergency medicine is highly desirable; experience with trauma centers or Level I/II trauma center environments is a plus.
- Strong knowledge of medical terminology, physiology, infection control practices, and aseptic techniques.
- Ability to perform comprehensive patient assessments and develop tailored care plans effectively.
- Excellent communication skills for triage calls and working with individuals with disabilities or complex medical needs.
- Certifications such as Basic Life Support (BLS), IV insertion, phlebotomy, and wound suturing are advantageous; additional skills in dialysis, cardiac catheterization support, or pain management are a plus.
