- Leading, mentoring, and supporting Direct Care Professionals to ensure consistent, high-quality care.
- Providing direct support with personal care, medication administration, daily living activities, and community engagement.
- Coordinating appointments, transportation, and activities that encourage participation in the community.
- Helping maintain accurate documentation and ensuring compliance with care plans, medical orders, and regulatory requirements.
- Supporting meal planning and preparation while ensuring individual dietary preferences and needs are respected.
- Participating in the development of Individual Plans of Service and advocating for the needs and goals of residents.
- Maintaining a safe, clean, and welcoming home environment.
- Assisting with quality assurance activities, emergency preparedness, medication audits, inventories, and other operational responsibilities that help the home run smoothly.

What You'll Bring
- Associate degree in a related field preferred, or a professional license such as CNA - not required.
- Previous experience in direct care, healthcare, or a related support environment preferred.
- Leadership, mentoring, or supervisory experience preferred.
- Strong communication, problem-solving, and organizational skills.
- A commitment to person-centered care and maintaining the highest standards of quality and compassion.

- Full-time, Afternoon Shift (3:00pm - 11:00pm)
- Monday - Friday
- On-call rotation approx. 1 or 2 weekends per month
- Overtime may be necessary based on operational and resident needs
