Wellness Care is launching a new care coordination platform focused on Chronic Care Management (CCM) and Transitional Care Management (TCM).
Our mission is both simple and ambitious: to prevent high-risk patients from “falling through the cracks” after hospital visits and to provide structured, ongoing support between appointments.
Position Overview: Initiating Visit RN
The Initiating Visit RN serves as the clinical entry point into our CCM program. This role conducts structured intake assessments, prepares patients for provider visits, and ensures a seamless handoff to the NP/MD for the clinical portion of care.
This is an in-person, call center–based RN role located in Richardson. It is a structured, process-driven position ideal for nurses who want to build expertise in care management and grow within a CCM-focused organization.
Compensation & Structure
- $45.00 per hour
- 1099 contractor engagement

- Consistent, scheduled in-office shifts
Key Responsibilities
- Call newly enrolled patients to complete structured intake assessments
- Collect required clinical and demographic information prior to provider visits
- Accurately complete the Basic Initiating Visit Intake Template
- Warm-transfer patients to the NP/MD via audio or Doxy video for the clinical visit
- Reconnect with patients after the provider visit to finalize the care plan
- Initiate and document CCM minutes
- Ensure all documentation meets CMS and CCM compliance standards
- Maintain professionalism, clarity, and patient confidence throughout onboarding

Qualifications
- Active RN license valid in all 50 U.S. states (required)
- 2 years of clinical nursing experience
- Strong communication skills and professional phone presence
- Highly detail-oriented with excellent documentation accuracy
- Comfortable working in a structured, high-volume call center environment
- Familiarity with CCM programs preferred
