What are the responsibilities and job description for the Case Manager - Jail/AODA Program position at Waushara County?
As a member of the Substance Abuse team, the Jail/AODA Case Manager will be responsible for the case management of clients once they are placed in custody and assist with the release planning in order to support them by becoming productive members of the community. This work involves extensive planning, organizing, coordinating and monitoring activities of clients in accordance with Human Services and Sherrif’s Department policies and procedures. This position will be responsible for the necessary community linkage and will work closely with the Substance Abuse Unit and the Sheriff’s Department to coordinate referrals and monitor client’s needs.
ESSENTIAL FUNCTIONS:
- Complete intake evaluations (Substance abuse screens TCU5) with all clients booked in Waushara County jail with a Substance Abuse history.
- Refer clients to contracted treatment agencies best suited for their needs. Collaborates with treatment providers to ensure community resources and services provided match desired program outcomes including Medication Assisted Treatment (MAT).
- Maintain a confidential filing system to facilitate the data collection necessary to monitor client’s compliance, identify trends, and provide a basis for ongoing evaluation of both the individual client and the program as a whole.
- Develop linkages with community agencies to address participant’s ancillary needs in the areas of medical, dental, educational, skills training, medically assisted treatment and employment training and placement.
- Document contacts and services, develop case plans and prepare reports and other written materials, and comply with all record-keeping requirements
- Provide crises and short-term intervention for clients
- Provide support for client’s families or significant others in individual, conjoint and group formats
- Assist clients with reaching goals developed on their individual case plans.
- Attend all relevant meetings and clinical staffing each week.
- Perform other duties as assigned
WORK ENVIRONMENT:
- Sedentary with occasional walking and standing in general office setting.
- Duties involve dealing with a variety of persons, some with emotionally stressful situations.
- Occasional travel is required.

- Demonstrated skill in completing written and oral presentations

- Knowledge and understanding of Wisconsin County, State court, and criminal justice systems, their agencies and their processes

- Ability to establish and maintain effective and appropriate relationships with a variety of persons and institutions, including ethnic minorities
- Ability to make public presentations
- Skill and proficiency with Microsoft Word and PowerPoint
- Must be able to work the allocated hours of the position
REQUIRED QUALIFICATIONS:
- A minimum of 1 year experience working with clients in the criminal justice system
- Certification / license with the State Department of Safety and Professional Services as any of the following:
· Substance Abuse Counselor in Training
· Substance Abuse Counselor
· Clinical Substances Abuse Counselor
- Must have access to private transportation at all times for work related duties, valid driver's license and insurance.
DESIRED LICENSES, CERTIFICATES AND OTHER REQUIREMENTS:
- Minimum of an associates degree from an accredited college or university with a major in Criminal Justice, Public Administration, Social Work, AODA or other closely related field
- WI Social Worker license
- Licensed Professional Counselor
DESIRED QULIFICATIONS:
- Bachelor’s degree from an accredited college or university with a major in Criminal Justice, Public Administration, Social Work, AODA or other closely related field
- Training and experience in Motivational Interviewing and achieve Substance Abuse Counselor certification within 5 years of hire
- WI Social Worker with a minimum of 2 years working in a treatment court or an AODA treatment facility
- Appointment will be conditional upon successful completion of criminal and caregiver background checks
Job Type: Full-time
Pay: $26.36 - $28.12 per hour
