What are the responsibilities and job description for the Care Delivery Coordinator position at WASHINGTON COUNTY MENTAL HEALTH SERVICES INC?
- Access & Pipeline Management: Support access-to-care timelines (such as time-to-first appointment and post-discharge follow-ups), monitor referral and intake pipelines for constraints, and propose operational solutions to leadership
- Provider Schedule & Template Administration: Build and maintain multi-site, multi-discipline, and telehealth provider schedules and templates in MyAvatar across individual, group, nursing, psychiatric, and care coordination service lines
- Schedule Deviation & Cancellation Resolution: Manage daily schedule adjustments, fill cancellation openings, place holds, approve overbooks within policy, and resolve schedule errors to prevent disruptions in client care
- Panel & Reassignment Tracking: Maintain client panel assignments and trackers in MyAvatar, execute reassignments based on provider leave or capacity, and flag uneven client loads or risks to the CCBHC Manager
- Cross-Team Workflow & Handoff Coordination: Standardize non-clinical handoff steps among front desk, clinical, and care management teams, trigger outreach workflows, and facilitate daily huddles and weekly access meetings
- Data Integrity & Operational Reporting: Produce routine performance reports—covering access timeliness, referral outcomes, no-show trends, and scheduling quality—while logging operational disruptions and maintaining data accuracy
- Compliance & Operational Execution: Handle protected health information in accordance with HIPAA and 42 CFR Part 2 standards, adhere to designated compliance pathways, and participate in rotating site coverage across four locations

The starting pay range for this position is $30.44-$31.68 per hour.
- Education: High School Diploma/GED required; Associate or Bachelor’s degree in healthcare administration, business, public health, or a related field (or equivalent experience) preferred
- Background in healthcare or behavioral health operations, scheduling, or clinic coordination preferred
- Proficiency in multi-site/multi-discipline scheduling and standard office software (email, spreadsheets, calendars)
- Strong operational problem-solving under time pressure, meticulous data quality and attention to detail, and knowledge of privacy rules (HIPAA and 42 CFR Part 2)
- Clear, assertive communication without direct authority
Workplace Conditions
Work occurs in office settings. Extended computer use while sitting or standing. Some evening and weekend hours required on a rotation to meet expanded clinical hours.
