Position Overview

The Physical Therapist (PT) is responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions designed to improve mobility, strength, balance, endurance, and overall function. The PT will work closely with physicians, nursing staff, therapy assistants, and families to ensure the highest quality of rehabilitative care.

Responsibilities

• Evaluate residents and develop individualized therapy treatment plans
• Provide physical therapy services in accordance with physician orders and regulatory guidelines
• Monitor resident progress and modify treatment plans as needed
• Collaborate with interdisciplinary team members regarding resident care and discharge planning
• Educate residents, families, and caregivers on treatment goals and exercises
• Maintain accurate and timely documentation
• Participate in care planning meetings and therapy team collaboration
• Ensure compliance with all state, federal, and company policies and procedures
• Promote a positive and encouraging rehabilitation environment

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Illinois Physical Therapist license required
• Previous skilled nursing or rehabilitation experience preferred, but not required
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Compassionate, patient-centered approach to care
