Job Title: Care Management Specialist
Location: Downers Grove, IL
Contract Duration: 6 months – Contract-to-Hire
Schedule: Monday–Friday | 8:00 AM – 5:00 PM
Overview
Responsible for discharge care coordination, episodic case management, and pre/post-discharge counseling for members with acute conditions. Supports safe care transitions, member education, and reduced readmissions.
Key Responsibilities
- Coordinate discharge planning & short-term case management
- Deliver pre-admission and post-discharge counseling
- Collaborate with physicians & discharge planners
- Educate patients on care plans, self-management & medications
- Arrange follow-up appointments & provider transitions
- Refer to community resources & internal departments as needed
- Support medication reconciliation & adherence
- Ensure compliance with licensure, documentation & HIPAA
Requirements
- RN or LMSW, active & unrestricted license
- 2 years clinical experience
- 1 year managed care/insurance experience
- Strong communication & customer-service skills
- Computer/database proficiency
- Occasional travel; valid driver’s license
