At Volunteers of America Eastern Washington & Northern Idaho, we do more than provide services—we help people recover, stabilize, and move forward. As a Care Coordinator (Case Manager equivalent role) in our Hope House Medical Respite program, you will support individuals recovering from illness or hospitalization while navigating homelessness and complex health needs. This is meaningful, fast-paced work where your coordination, advocacy, and compassion directly change lives.  


What You’ll Do

  • Assess participant needs across medical, behavioral health, housing, and social services
  • Develop and coordinate individualized care plans with medical and community providers
  • Support transitions from medical respite to housing or ongoing care
  • Connect participants to resources, including healthcare, benefits, housing, and community services
  • Coordinate appointments, referrals, and provider communication
  • Advocate to reduce barriers and increase engagement in care
  • Maintain accurate, timely documentation in the EHR system
  • Participate in interdisciplinary care team meetings

Schedule

  • Full-time (40 hours/week)
  • Thursday through Sunday 9:00 am - 7:00 pm (4x10's)


Requirements

What You Bring

Required

  • Experience in case management, care coordination, healthcare, or human services
  • Experience supporting individuals with complex needs (e.g., homelessness, behavioral health, substance use)
  • Strong organizational skills and ability to manage multiple priorities
  • Ability to coordinate across healthcare and community systems
  • Ability to obtain an Agency Affiliated Counselor (AAC) credential within 30 days
  • Ability to pass background and exclusion checks

Preferred

  • Bachelor’s degree in Social Work, Human Services, Public Health, or related field
  • Experience in medical respite or healthcare-focused coordination
  • Knowledge of housing and homelessness systems
  • Familiarity with Medicaid or healthcare navigation
