We are seeking a compassionate and experienced Social Worker to join our multidisciplinary team.
About the Role:
- Provide social work services to our residents, including assessments, treatment planning, counseling, and education
- Develop and implement individualized care plans in collaboration with our interdisciplinary team
- Identify and address the social and emotional needs of our residents and their families
- Facilitate communication and coordination with our medical staff, rehabilitation therapists, and other healthcare professionals
- Participate in care conferences and other meetings to ensure that our residents receive comprehensive and coordinated care
Responsibilities:
- Conduct initial and ongoing assessments of our residents' social and emotional needs
- Develop and update individualized care plans to address identified needs and goals
- Provide counseling, support, and education to our residents and their families
- Collaborate with our medical staff and rehabilitation therapists to ensure comprehensive care
- Facilitate communication between our residents, their families, and our interdisciplinary team
- Participate in quality improvement initiatives to enhance our services and outcomes
Requirements:
- Bachelor's degree in Social Work or related field
- Minimum of two years of experience in social work, preferably in a healthcare setting
- Licensure as a Social Worker in the State of Maryland (or ability to obtain)
