As a Social Worker at Potomac Valley Rehabilitation and Healthcare Center, you will play a vital role in supporting our residents and their families in achieving their physical, emotional, and social well-being. Our 165-bed facility is well-regarded for its high-quality rehabilitation, skilled nursing, palliative care, and behavioral health programs. We are seeking a compassionate and experienced Social Worker to join our multidisciplinary team.

About the Role:

• Provide social work services to our residents, including assessments, treatment planning, counseling, and education

• Develop and implement individualized care plans in collaboration with our interdisciplinary team

• Identify and address the social and emotional needs of our residents and their families

• Facilitate communication and coordination with our medical staff, rehabilitation therapists, and other healthcare professionals

• Participate in care conferences and other meetings to ensure that our residents receive comprehensive and coordinated care

Responsibilities:

• Conduct initial and ongoing assessments of our residents' social and emotional needs

• Develop and update individualized care plans to address identified needs and goals

• Provide counseling, support, and education to our residents and their families

• Collaborate with our medical staff and rehabilitation therapists to ensure comprehensive care

• Facilitate communication between our residents, their families, and our interdisciplinary team

• Participate in quality improvement initiatives to enhance our services and outcomes

Requirements:

• Bachelor's degree in Social Work or related field

• Minimum of two years of experience in social work, preferably in a healthcare setting

• Licensure as a Social Worker in the State of Maryland (or ability to obtain)
