As a Registered Dietitian, you’ll be on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.
Your Impact:
As a Registered Dietitian you will provide nutritional assessments, education, and consultation for patients in our Hospice House and community settings.
You Will:
Assess nutritional needs for patients in the Hospice House and make recommendations as part of the interdisciplinary team.
Provide home visits for hospice and certified home health agency (CHHA) patients, offering individualized dietary education and support.
Assist with kitchen oversight in the Hospice House, ensuring compliance with NYS regulations (temperature tracking, food safety, supply levels).
Educate staff and volunteers on safe and therapeutic food preparation.
Collaborate with nurses, social workers, and other clinicians to manage patient nutrition plans.
Document assessments and recommendations within 24 hours of evaluation.
Attend Care Management and Interdisciplinary Team (IDT) meetings.
Contribute to staff training and patient education materials on therapeutic diets and current nutrition trends.
What You Bring:
Registered Dietitian (RD) through the Commission on Dietetic Registration
Certified Dietitian/Nutritionist (CDN) in New York State
Experience in home care, hospice, or community-based healthcare preferred
Excellent communication, time management, and documentation skills
Ability to work independently and collaboratively with clinical teams
