As a Team Assistant, you’ll play a vital role in supporting clinical staff and ensuring patients receive timely, high-quality services.
Your Impact:
The Team Assistant provides essential clerical and administrative support to an interdisciplinary clinical team, ensuring efficient scheduling, communication, and coordination of patient care services.
You Will:
Scheduling & Coordination
Schedule all new patient visits and assign case managers in a timely manner.
Coordinate skilled service visits and adjust schedules to meet patient needs.
Assist in covering unplanned visits and ensure continuity of care.
Prepare and finalize weekly schedules, including LPN coverage and weekend planning.
Team Support & Communication
Serve as the front-line contact for incoming calls and team communications.
Maintain ongoing communication with field staff regarding schedule updates and patient needs.
Prepare and distribute reports and materials for care management meetings.
Maintain time-off calendars and assist with staffing coverage.
Operational & Administrative Support
Track and monitor staff schedules to ensure productivity goals are met.
Maintain and organize patient documentation, including SOC (Start of Care) paperwork.
Track timeliness of clinical documentation and follow up as needed.
Support coordination of team activities as directed by the Clinical Manager.
Compliance & Organization
Ensure all documentation is received, tracked, and filed accurately.
Maintain confidentiality and comply with all agency policies and regulations.
Support overall team operations with strong organization and attention to detail.
What You Bring:
High School Diploma or equivalent required (LPN license a plus).
Minimum 2 years of office experience, preferably in a healthcare setting, or LPN experience.
Knowledge of medical terminology.
Strong computer and data entry skills.
Excellent organizational, communication, and customer service abilities.
Ability to multitask and work in a fast-paced environment.
