About the role
The Physical Therapist Assistant supports patients in their homes by implementing individualized therapy plans created by the supervising Physical Therapist. This role focuses on restoring mobility, improving functional independence, reducing pain, and enhancing quality of life for patients receiving home care services or hospice care.
You’ll work one‑on‑one with patients, helping them stay safe, comfortable, and as independent as possible in their preferred environment.
What You’ll Do
- Provide therapeutic interventions — Deliver exercises, gait training, strengthening, balance activities, and mobility support per the PT’s plan of care.
- Monitor patient progress — Observe, document, and report changes in condition, functional ability, and response to treatment.
- Educate patients & caregivers — Teach safe mobility techniques, fall‑prevention strategies, and home exercise programs.
- Support hospice goals — Focus on comfort, positioning, gentle mobility, and quality‑of‑life interventions for hospice patients.
- Collaborate with the interdisciplinary team — Communicate with PTs, nurses, aides, and social workers to ensure coordinated care.
- Maintain accurate documentation — Complete timely, compliant visit notes in the EMR.
- Ensure safety in the home — Identify environmental risks and recommend modifications to support mobility and independence.
- Improve or maintain patient mobility and functional independence
- Reduce fall risk and promote safety in the home
- Support comfort and quality of life for hospice patients
- Provide compassionate, evidence‑based care aligned with patient goals
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Active PTA license in the state of practice (NH)
- Valid driver’s license and reliable transportation for home visits
- Experience in home health, outpatient, rehab, or hospice preferred
- Strong communication, observation, and patient‑teaching skills
- Ability to work independently in varied home environments
- Compassionate, patient‑centered approach to care
- Comfort working with older adults, chronic illness, and end‑of‑life needs
- Strong problem‑solving and clinical judgment
- Ability to adapt treatment to unique home settings
- Excellent time‑management and organizational skills
Work Environment
- Field‑based role with daily travel to patient homes
- Flexible, autonomous schedule with supportive clinical oversight
- Collaboration with PTs, RNs, OTs, social workers, and aides
- Exposure to diverse home environments and patient needs
