Licensed Practical Nurse – Home Care & Hospice
Location: North Conway, NH
Schedule: Offering
Part-time: 3 days per week, Monday, Wednesday, and Friday
Full-time: 5 days per week, Monday through Friday

About the Role
As a Licensed Practical Nurse (LPN) in our Home Care & Hospice team, you will be more than a clinician; you will be a trusted presence in some of life’s most vulnerable moments. Your care will help patients maintain dignity, independence, and comfort in their own homes. You’ll be a vital link between patients, families, and the broader care team, ensuring that every individual receives the attention, compassion, and expertise they deserve.
What You'll Do
- Provide skilled nursing care in patients’ homes, including medication administration, wound care, catheter care, and symptom management.
- Monitor and document patient conditions, promptly reporting changes to the RN Case Manager or physician.
- Offer emotional support and education to patients and families, helping them understand care plans and manage symptoms.
- Collaborate closely with interdisciplinary team members—RNs, social workers, chaplains, and therapists—to ensure holistic care.
- Assist with end-of-life comfort measures, honoring patient wishes and supporting families through the hospice journey.
- Maintain accurate, timely clinical documentation in compliance with regulatory and agency standards.
Qualifications
- Current, active LPN license in the states of NH and Maine (employer will assist in applying for Maine License).
- At least 1 year of nursing experience; home health or hospice experience preferred.
- Strong clinical assessment skills and the ability to work independently in varied home environments.
- Compassion, patience, and the ability to build trust quickly with patients and families.
- Reliable transportation and a valid driver’s license.
- Comfort with electronic medical records and mobile documentation tools.
