In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $35,000.0 to USD $60,000.00. When determining an offer, VNACJ CHC considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits. What You’ll Do
Community Engagement & Outreach
- Build relationships with community partners, local organizations, and residents.
- Participate in and help plan health fairs, screenings, community events, and coalition activities.
- Provide direct outreach to targeted populations and connect individuals to VNACJ CHC for primary and dental care.
- Support marketing and outreach efforts by creating promotional materials (flyers, social media graphics, etc.) in Canva and promoting events across social media and the organization’s website.
- Assist patients with eligibility, enrollment, and understanding health insurance and community resources.
- Enroll families in Children and Family Health Institute programs and make referrals to community‑based services.
- Collaborate with transportation providers, counselors, translators, and other service partners to reduce barriers to care.
- Provide backup support to front desk, call center, and patient navigation teams.
- Maintain accurate documentation, data collection, and reporting for grant requirements.
- Promote a positive, safe, and efficient environment by following VNACJ CHC policies and procedures.
- Support a mission‑driven culture centered on strong relationships with patients, colleagues, and the community.
- High school diploma/GED required; Associate’s or Bachelor’s degree in health education or social work preferred
- Bilingual English/Spanish strongly preferred
- 2–3 years of community outreach or related experience
- Strong computer skills and comfort learning new systems
- Excellent communication, customer service, and interpersonal skills
- Knowledge of health insurance programs (Medicare, Medicaid, Marketplace) preferred
- Familiarity with community health centers, human services, or immigration‑related services is a plus
- Ability to complete the NJ Department of Health Community Health Worker Training (or equivalent) within the first 6 months of employment
- Ability to perform effectively in a fast‑paced environment and collaborate with patients and staff to meet deadlines
- Valid driver’s license, auto insurance, and a registered, reliable vehicle
