Position Overview
The RN Case Manager is responsible for overseeing the clinical care of Visiting Angels clients, ensuring that individualized care plans are developed, implemented, and maintained to the highest standard. This role serves as the clinical liaison between clients, families, caregivers, and office staff to ensure safe, effective, and compassionate home care. The RN Case Manager plays a critical role in supporting seniors and veterans to live independently at home by providing professional nursing oversight and ensuring compliance with Visiting Angels policies, state regulations, and best practices in home care.
Key Responsibilities
Clinical Assessment & Care Planning
Conduct initial nursing assessments for new clients, evaluating medical history, functional status, and care needs.
Develop individualized care plans that reflect client goals, physician orders, and regulatory requirements.
Update and revise care plans as client conditions or preferences change.
Perform supervisory visits in clients’ homes to evaluate caregiver performance, client satisfaction, and quality of care.
Case Management & Oversight
Serve as the primary clinical contact for assigned clients and their families.
Monitor the delivery of care to ensure alignment with care plans and professional standards.
Communicate with physicians, healthcare providers, and family members regarding client status and changes.
Coordinate with the office scheduling team to ensure continuity of care.
Caregiver Support & Training
Provide orientation and ongoing training to caregivers on client-specific care needs, disease management, infection control, and safety protocols.
Mentor caregivers to promote professional growth and quality performance.
Evaluate caregiver competency during supervisory visits and provide feedback or corrective action when necessary.
Compliance & Documentation
Maintain accurate and timely documentation of all assessments, supervisory visits, and care plan updates in accordance with agency policies and state regulations.
Ensure adherence to Visiting Angels’ policies, procedures, and compliance with state licensing requirements.
Participate in quality assurance programs and contribute to performance improvement initiatives.
Team Collaboration
Partner with the Client Care Coordinators, HR team, and Scheduling Coordinators to align caregiver assignments with client needs.
Provide clinical input during intake, client matching, and service expansion.
Participate in team meetings to review client care and agency operations.
Qualifications
Current Registered Nurse (RN) license in the state of Ohio (in good standing).
Minimum 2 years of nursing experience; home health, hospice, or geriatric care experience strongly preferred.
Strong assessment, critical thinking, and problem-solving skills.
Excellent communication and interpersonal abilities with clients, families, caregivers, and colleagues.
Ability to manage multiple cases while maintaining attention to detail and timeliness.
Proficiency with electronic health records and standard office technology.
Valid driver’s license, reliable transportation, and willingness to travel locally for home visits.
