What are the responsibilities and job description for the Home Health Registered Nurse (RN) - Case Manager position at Alta Home Health Care LLC?
We are looking for a Registered Nurse (RN) to join our Home Health team as a Case Manager. The ideal candidate will have experience delivering direct patient care in a home health setting, including conducting assessments, creating and implementing care plans, educating patients, and collaborating with other healthcare providers. The successful candidate will be a compassionate licensed Registered Nurse (RN), have strong communication skills, and be able to deliver high-quality care in a fast-paced environment.
Responsibilities:
- Conduct patient assessments and administer treatments as outlined in the care plan.
- Educate patients and assist with discharge planning.
- Document patient progress and care in compliance with agency policies and procedures.
- Monitor vital signs, administer medications, and provide wound care as required.
- Communicate with physicians, family members, and other healthcare professionals to ensure seamless care.
- Uphold patient confidentiality in line with HIPAA regulations.
Compensation:
- Registered Nurses are paid per visit. Visits are 35 minutes min. $50 for a Recertification, and $60 for a Start of Care or Resumption of Care.
- Additional Nursing Visits available to pick up.
- Mileage Reimbursement
