Job Summary
We are seeking compassionate and dedicated Community Support Team Qualified Professional to join our team in providing exceptional care to individuals in various settings, including home environments, group homes and assisted living facilities. The ideal candidate will possess a strong commitment to enhancing the quality of life for our clients through direct support while providing Community Support Team services to assist adults with mental illness, substance abuse disorders, or both who have complex and extensive treatment needs in achieving rehabilitative and recovery goals. This role requires a blend of patience, and the ability to work effectively with individuals with diverse needs.
Responsibilities
- Provide psychoeducation as indicated in the Person Centered Plan.
- Assists with crisis interventions.
- Assist the Team Lead with behavioral and substance use disorder treatment interventions
- Assists with the development of relapse prevention and disease management strategies
- Participates in the initial development, implementation and ongoing revision of the Person Centered Plan
- Monitoring and documenting the status of the beneficiary’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan
- Providing intensive case management
- Ensuring linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
- Monitoring and documenting the status of the beneficiary’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan
- Completing functional needs assessment(s) to determine the scope and anticipated outcomes to the services
- Communicates the recipient's progress and the effectiveness of the strategies and interventions to the Team Leader as outlined in the Person Centered Plan.
Experience
Preferred candidates will have experience in the following areas:
- Must have a minimum of a bachelor degree and 1 year experience working with beneficiaries with mental health or substance use disorders and have the knowledge, skills and abilities required by the population and age to be served
- The qualified professional shall meet the requirements specified, according to 10A NCAC 27G.0104 (12)
- Ability to provide services as defined by service definitions, County Program/Area Authority/Local Management Entity guidelines and NC Division of MH/DD/SAS rules and regulations
- Must successfully complete the minimum training specific to the required components of the Community Support Team service definition within the required days of hire
- High level of professionalism
- Excellent written and oral communication skills.
- Valid driver's license and ability to obtain and maintain appropriate car insurance coverage.
- Must be on call and accessible during periods of crisis for clients receiving services and available for face-to-face contact within two (2) hours of emergency/crisis notification
- Ability to work independently and as part of a team
- Flexibility in responding and adjusting to change
- Ability to prioritize and manage time.
- Advocacy and negotiation skills.
- Must have positive attitude and view the individual as first priority.
- First aid and CPR certification
