About the Role
We’re seeking a mission-driven Qualified Mental Health Professional (QMHP) to deliver Mental Health Skill-Building Services (MHSS) to adults in community settings across the Tidewater area. You’ll provide person-centered, recovery-oriented, and trauma-informed supports that help adults increase independence, maintain stability in the community, and reduce hospitalizations.
Key Responsibilities
- Provide face-to-face skill-building interventions in the community and within and around the home when appropriate (e.g., ADLs/IADLs, medication management education, coping skills, transportation training, budgeting, and community integration).
- Conduct intake screenings, strengths-based assessments, person-centered ISP development, and ongoing service plan reviews.
- Deliver structured sessions aligned with medical necessity and Medicaid/insurer guidelines; maintain service intensity and caseload expectations.
- Coordinate care with CSBs, MCOs, primary care, housing providers, probation/parole, and natural supports; participate in treatment team and case conferencing.
- Monitor risk, crisis-plan proactively, and respond to crises using least restrictive interventions; complete mandated incident reporting.
- Document all contacts timely and accurately in the EHR (progress notes, ISPs, discharge summaries within LAURIS) meeting DMAS/DBHDS/MCO standards.
- Track outcomes (hospitalizations, housing stability, employment/education linkages, functional gains) and contribute to quality improvement.
Minimum Qualifications
- Registered with Virginia DBHDS as QMHP.
- Bachelor’s degree in human services (e.g., Social Work, Psychology, Counseling, Special Ed, Human Services) or equivalent per DBHDS criteria.
- One (1) year relevant experience providing behavioral health services (community-based preferred).
- Valid Virginia driver’s license, reliable insured vehicle, and ability to travel throughout Tidewater.
- Ability to pass background checks, (fingerprinting and Central Registry) exclusion screening, and meet TB, CPR/First Aid requirements.
- Strong writing/EMR skills; professionalism, cultural humility, and commitment to trauma-informed care.
Preferred Qualifications
- Experience with MHSS, serious mental illness (SMI), co-occurring substance use, justice-involved populations, and housing/benefits navigation.
- Familiarity with Medicaid (DMAS/MCO) documentation standards, service authorization, and utilization management.
Schedule & Work Environment
- Flexible scheduling with a mix of daytime/early evening hours to meet member needs; some weekends as needed.
- Field-based role with travel across assigned Tidewater localities.
