Complete Work History Employer/School District name for each position Specific Position Title Dates of Employment Assignment Information (e.g., Grade level, subject taught, etc.) b. Credential(s) held c. Specialized/Supplemental trainings/Authorizations d. Education 4. Letters of Recommendation (2 required) Must be dated within the past year 5. EL Certification Additional Requirements for Applicants without an Appropriate Credential: 1. Letter of Introduction Letter must include: a. Type of emergency credential in which you qualify b. University program details c. Expected credential recommendation/title d. Expected program completion date 2.
