The Social Worker / Therapist provides comprehensive assessments, evidence-based treatment, and individualized aftercare planning for residents in a residential setting addressing co-occurring mental health and substance use disorders.

• Delivers clinical care, leads therapy groups, and develops treatment plans that meet licensing standards.
• Coordinates care, collaborates with referral resources, and ensures continuity post-discharge.
• Maintains accurate documentation, promotes recovery-oriented interventions, and supports staff development.
