RN Case Manager / Social Worker - Full Time

Role: Full-time RN Case Manager or Social Worker responsible for coordinating patient care from admission to discharge, including assessment, planning, collaboration, intervention, monitoring, and evaluation.

Responsibilities

• Perform care management to assess and facilitate discharge planning, documenting assessments within 48 hours of admission.
• Use Interqual criteria to evaluate appropriateness and level of care, conducting ongoing assessments and re-evaluations.
• Document avoidable days and coordinate with leadership for interventions.
• Ensure safe discharge planning and post-acute care services through collaboration with healthcare team members.
• Provide patient and family education on disease management and follow-up care.
  
  

Qualifications & Skills

• Current Nevada RN or Social Worker license.
• Minimum five years of nursing experience and two years in utilization review or case management preferred.
• Discharge planning experience is preferred.
• Current BLS certification.
