- Conduct patient assessments to determine functional needs and treatment goals.
- Develop and implement individualized treatment plans based on clinical evidence.
- Collaborate with physicians and an interdisciplinary care team to coordinate care.
- Educate patients and families on home exercise programs and self-management.
- Maintain accurate documentation of evaluations and treatment in the EMR.
- Licensure/credential as a Physical Therapist.
- Strong clinical reasoning and ability to design effective rehab programs.
- Excellent communication and patient/family education skills.
- Comfort working in a team-based, interdisciplinary environment.
- Proficiency with electronic medical records for care documentation.
