- Conduct comprehensive patient assessments and determine functional needs.
- Create individualized care plans and deliver physical therapy interventions.
- Collaborate with physicians and an interdisciplinary care team.
- Educate patients and families on home exercise programs and self-management.
- Document evaluations, treatments, and progress in the EMR.
- Licensed Physical Therapist with strong clinical assessment and treatment planning skills.
- Ability to apply evidence-based practice across diverse patient diagnoses.
- Excellent communication and patient/caregiver education skills.
- Strong teamwork and collaboration with multidisciplinary providers.
- Proficiency with electronic medical records for accurate documentation.
