Nurse Practitioner - Pulmonary - SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care to provide high-quality, patient-centered services in skilled nursing and long-term rehabilitation settings.

• Manage complex respiratory conditions, including COPD, asthma, pneumonia, and respiratory failure.
• Perform comprehensive assessments, diagnose, and treat pulmonary issues, including ventilator and tracheostomy management.
• Coordinate care with multidisciplinary teams, educate patients and families, and participate in care planning and hospital readmission reduction initiatives.
• Complete timely documentation and ensure compliance with regulations.
• Work 3 days/week, including weekends, with occasional on-call coverage.
  
  

Qualifications include active APRN license, Master's degree, experience in long-term care, BLS/ACLS, and strong clinical skills. Compensation is productivity-based as a part-time contractor with benefits, flexible schedule, and ongoing professional development opportunities. This in-person role is based in Harrisburg, PA.
