Nurse Practitioner - Pulmonary - SNF / Long-Term Care

We seek an experienced APRN in pulmonary care for in-person, part-time work in skilled nursing and long-term rehab settings, focusing on managing complex respiratory conditions.

• Provide pulmonary and acute care, including admissions, discharges, assessments, and management of conditions like COPD, asthma, and respiratory failure.
• Manage noninvasive ventilation therapies (CPAP/BiPAP), tracheostomy care, and coordinate ventilator weaning plans.
• Prescribe medications, interpret diagnostics, monitor treatments, and collaborate with interdisciplinary teams.
• Educate patients and staff, participate in initiatives to reduce hospital readmissions, and ensure regulatory compliance.
• Requires active APRN license, experience in tracheostomy care, BLS/ACLS, and strong clinical skills.
