We are seeking an experienced and compassionate RN Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations, ensuring the highest quality of patient care and maintaining regulatory compliance within our home health agency.

- Develops, plans, implements, analyzes, and organizes clinical operations.
- Supervise and support clinical staff including RNs, LPNs, therapists, aides, and other home health professionals.
- Review and approve patient assessments, care plans, and documentation.
- Conduct routine case conferences and participate in interdisciplinary team meetings.
- Monitor clinical performance, caseloads, and patient outcomes.
- Provide field supervision and performance evaluations as needed.
- Collaborate with the intake, scheduling, and administrative teams to ensure efficient operations.
- Assist in hiring, onboarding, and training new clinical staff.
- Participate in quality improvement initiatives and ensure documentation accuracy.
- Serve as a liaison between clinical staff, physicians, patients, and families.
- Graduate of an accredited School of Nursing.
- Registered Nurse (RN) license in good standing, required.
- Clinical leadership experience in a Medicare-certified home health agency.
- Strong understanding of PDGM, QAPI and care coordination.
- Passion for delivering high-quality patient-centered care in a home setting.
- Must know OASIS and two years as a Registered Nurse with at least one year of management experience in home care.
