Appleton Home Health is seeking a compassionate, organized, and motivated Registered Nurse to join our team as a Home Health Case Manager.

We invest in our team with Blue Cross PPO health insurance with 100% of the employee premium paid by the company and a SIMPLE IRA with a dollar-for-dollar company match on employee contributions up to 3% of compensation.

This position is ideal for an RN who enjoys coordinating care, communicating with patients and healthcare professionals, and helping individuals remain safely in their homes. Previous home health case management experience is preferred but not required. We are willing to train the right candidate who has clinical experience, strong communication skills, and a desire to learn.

Responsibilities

• Coordinate and manage patient care services.
• Communicate with physicians, nurses, therapists, patients, and family members.
• Assist with care planning and coordination of services.
• Review and maintain patient documentation for accuracy and completeness.
• Obtain and track physician orders.
• Coordinate admissions, recertifications, and care transitions.
• Monitor patient progress and follow up on care needs.
• Support quality assurance and regulatory compliance activities.
• Provide responsive, professional service to patients, referral sources, and healthcare partners.
• Work closely with the clinical and administrative team to support quality patient care.

Qualifications

• Active Florida Registered Nurse (RN) license required.
• One year of nursing experience preferred.
• Home health or case management experience is a plus but not required.
• Strong communication and organizational skills.
• Comfortable using computers and electronic medical records.
• Ability to multitask and manage competing priorities.
• Professional, dependable, and detail-oriented.
• Willingness to learn and grow within the organization.

Work Location

This is an on-site position working from our office and collaborating closely with clinical staff, physicians, referral sources, and patients.
