Appleton Home Health is seeking a compassionate, organized, and motivated Licensed Practical Nurse to join our team as a Home Health Care Coordinator.

This position is ideal for an LPN who enjoys coordinating services, communicating with patients and healthcare professionals, and helping individuals remain safely in their homes. You will work closely with our RN case managers and clinical leadership to support patient care, documentation, and daily operations.
Previous home health experience is preferred but not required. We are willing to train the right candidate who has clinical experience, strong communication skills, and a desire to learn.

Responsibilities
- Support RN case managers with patient care coordination and follow-up.
- Communicate with physicians, nurses, therapists, patients, and family members.
- Assist with coordinating services according to established care plans.
- Review patient records for missing documentation and follow up with the appropriate team members.
- Assist with obtaining and tracking physician orders under established procedures.
- Support the coordination of admissions, recertifications, and care transitions.
- Follow up with patients and caregivers, documenting and reporting care concerns to the supervising RN.
- Assist with quality assurance and compliance activities.
- Provide responsive, professional service to patients, referral sources, and healthcare partners.
- Perform assigned nursing responsibilities within the LPN scope of practice and under appropriate clinical direction.
Qualifications
- Active Florida Licensed Practical Nurse (LPN) license required.
- One year of nursing experience preferred.
- Home health or care coordination experience is a plus but not required.
- Strong communication and organizational skills.
- Comfortable using computers and electronic medical records.
- Ability to multitask and manage competing priorities.
- Professional, dependable, and detail-oriented.
- Willingness to learn and grow within the organization.
Work Location
This is an on-site position working from our office and collaborating closely with RN case managers, clinical staff, physicians, referral sources, and patients.
