Vertus HealthCare is seeking experienced Registered Nurses (RNs) to provide quality skilled nursing care to patients in the comfort of their homes throughout Palm Beach County.

- Provide skilled nursing visits based on the patient’s plan of care.
- Complete OASIS, SOC, ROC, Recertification, and Discharge visits as assigned.
- Assess patient needs and coordinate care with the clinical team.
- Complete timely and accurate clinical documentation.
- Communicate with physicians, patients, families, and the healthcare team.
- Current Florida RN license.
- Home Health experience preferred.
- OASIS experience preferred.
- Strong clinical assessment and documentation skills.
- Reliable transportation.
